This section requires more detailed information about your organization, structure, and compliance, and may take a bit more time to complete. It helps our review committee fully understand your capacity, accountability, and readiness to manage funding.

We recommend setting aside some uninterrupted time to complete this section. Answer questions as clearly as possible and upload any supporting documents. If something doesn’t apply, just note “N/A.”

 
ORGANIZATION INFORMATION
Is your organization recognized by the United States Internal Revenue Service (IRS) as a non-profit / not for profit 501(c)(3) public charity?
Is your organization in good standing with IRS Form 990 filings?
Is your organization in good standing with state charitable registration requirements?
Has your organization had its tax-exempt status revoked, suspended, or under review within the last 5 years?

Is your organization recognized as a public charity by an official (government) entity in a country other than the United States? 

Please provide support documents confirming registration and current status

Do you have a financial sponsor or supporter for this project?

COMPLIANCE / RISK MANAGMENT
Will any portion of the requested funds be used for:
Influencing laws or government policy?

Religious instruction or promotion of a particular faith?
Grants to individuals?
Re-granting / re-gifting to other organizations?
